Abstract     Introduction: Magnetic resonance imaging (MRI) is considered to be the gold standard for preoperative diagnosis, local staging and post therapeutic monitoring for brain tumours.1,2,3 In many instances, reliable differentiation of neoplastic from non- neoplastic brain lesions or of high-grade from low-grade tumours, is difficult with conventional MRI. Aims and Objectives: To Study of space occupying lesions of brain with respect to Signal intensity characteristics. Methodology: This was a cross sectional observational study of 50 patients done in Radiology department of tertiary care hospital. Institutional ethics committee clearance and approval of the MUHS were obtained prior to the study. We studied 50 patients; who were found to have focal intra cranial pathologies on routine MRI from 1st December 2012 - 31st October 2014.The study was carried out on GE SIGNA HDX 1.5 Tesla MRI MACHINE. Single and multi-voxel Spectroscopy performed on the various intracranial lesions as per requirement and data analysis was done. Patients referred by clinical departments whose MRI study revealed space occupying lesions in brain included into study. Result: In present study, maximum numbers of patients were in the age group of 51-60 yrs. (36 %).In our study of 50 patients, 31 (62%) patients were males and 19(38 %) were females. Majority of the Space occupying lesions were primarily neoplastic (Benign and malignant) 60%, Infective and inflammatory 36 %, secondarily neoplastic (Metastasis) 4 %Most of the lesions were hypo intense on T1WI and ISO to hyper intense on T2WI. High grade neoplasm with intra tumoural bleed, cases with some calcified granulomas and calcified meningioma showed blooming on gradient.20 % patients with space occupying lesions showed Blooming and 80 % not shown the Blooming. Conclusion: MR spectroscopy findings are very helpful in differentiating tuberculous granulomas/ abscesses from neoplasm’s in patients with ring enhancing lesions. Hypointensity on T2WI with partial or complete restriction on DWI images and lipid peak on MRS is more in favour of Tuberculoma.
